MIDI In/Out is used on MPP as a way to control the piano with external software or real-world instruments and vice versa. MPP has support for any size keyboard up to 88-key keyboards as well as sustain. MIDI is not the primary system used for note messages, as all MIDI messages are converted to/from MPP's note message.
